Terms and Conditions
$35 Rental Fee for non-majors and for music minors*
By submitting this form, I understand that the Colorado State University School of Music, Theatre, and Dance will charge my account $35 for single locker use every semester if I am NOT enrolled as a music major OR as a music minor until I clear out my locker (required at the end of spring semester). NOTE: shared lockers and additional lockers will affect the amount charged to your student account.

Additional information about the locker fee*
By submitting this form, I understand that lockers are provided at no cost to CSU students who are registered as music majors OR as music minors who also pay the music program fee for use during the current semester. If you are a non-major or not paying the music program fee, you will be charged a $35/semester fee for locker use.

University policy about opening lockers*
By submitting this form, I understand that the Colorado State University School of Music, Theatre, and Dance reserves the right to open any locker should appropriate circumstances arise in accordance with University policy.

Modifications to lockers*
By submitting this form, I understand that my locker assignment is a lease issued by the Colorado State University School of Music, Theatre, and Dance, and may be modified/altered to best accommodate the needs of the School, which may include sharing a locker space. Locker rentals are the property of the UCA and I will not cause damage to the exterior/interior of the lockers. All items, including trash, must be removed from the locker when the locker rental period ends or I may be subject to additional fees.

Liability and Insurance*
The Colorado State University School of Music, Theatre, and Dance, and its faculty and staff, are not liable for any lost, stolen, or damaged property stored in a locker. I understand that it is my responsibility to insure my property, and by entering into this locker rental agreement, I release CSU and its employees from all liability.

By submitting this form, I understand that lockers are designated as instrument storage lockers and I am not guaranteed space for personal items such as books, music, backpacks, jackets, etc. NOTE: if you would like to request an additional locker for an additional fee, please visit the SMTD front office. Food*
By submitting this form, I understand that lockers are not to be used to store food items. These lockers are designated as instrument storage lockers and not airtight. Unattended food may attract pests and clean up for these damages will be my responsibility. If food is found in my locker, I might lose the ability to request a locker in future semesters.

Liability and Insurance*
By submitting this form, I understand that if I lose a lock that belongs to the School of Music, Theatre, and Dance, I will be charged a $5 replacement fee.
