Instrument Rental Request Form

Please fill out the form below to use a CSU instrument for the Summer 2026 semester.

PLEASE NOTE:

Instruments may only be used by CSU students (not just music majors or minors) for use in CSU ensembles, methods courses, or applied lessons. There is a $50 fee for instrument rentals for students not enrolled as music majors in these courses during the current semester. Summer rentals are for music majors and minors only.
Please allow 2-3 days for a response from our instrument curator, Megan Bellamy-Lanz, who will provide the necessary details to obtain your requested instrument.
All rentals must be returned to their assigned locker by the date listed on your rental form. All rentals during the school year are valid through juries.
In rare cases, you may be asked to share an instrument with another student. This will be assessed on a case-by-case basis.

Note: Students will fill out a different form for all athletic band instrument rentals (Marching Band, Basketball Pep Band, and Presidential Pep Band). Please don't fill this form out for athletic band rentals.

Instrument Request

This request is for one instrument only. Any additional requests require that you fill out an additional form. You may request a specific instrument from our inventory (i.e., "Trombone 3") however, these requests are not always possible to fulfill.

I understand that this instrument is being checked out under my name and my name only and I am solely responsible for the instrument.

I understand that all necessary repairs must be brought to the immediate attention of the instrument curator and the damage will be subject to the appropriate repair procedure.

I understand that I am financially responsible for the return of the instrument and all of its included accessories in good condition or replacement of the instrument or part(s) will be billed to my student account. This includes loss and damages incurred from unreasonable or accidental wear and tear or theft.

I understand that if I choose to share my locker combination with anyone I am still solely and financially responsible for the instrument and all accessories.

I understand that the term of this contract shall begin on its signing date and extend until August 15, 2026.

I understand that the instrument must be returned by August 15, 2026, or my university account will be charged the replacement value of the instrument and any included accessories.
