Instrument Rental Request Form

I understand that CSU will charge my student account $50 for this one-semester instrument rental if I'm not enrolled as a music major or minor for the current semester.

I understand that this instrument is being checked out under my name and my name only and I am solely responsible for the instrument.

I understand that all necessary repairs must be brought to the immediate attention of the instrument curator and the damage will be subject to the appropriate repair procedure.

I understand that I am financially responsible for the return of the instrument and all of its included accessories in good condition or replacement of the instrument or part(s) will be billed to my student account. This includes loss and damages incurred from unreasonable or accidental wear and tear or theft.

I understand that locker combinations are not to be shared, and if I choose to share my locker combination with anyone I am still solely and financially responsible for the instrument and all accessories.

I understand that the term of this contract shall begin on its signing date and extend until December 18, 2026.

I understand that the instrument must be returned by December 18, 2026, or my university account will be charged the replacement value of the instrument and any included accessories.
